Foundation of Data Sharing

I thought, this time let me cover the importance of user consent in data exchange and provides a boilerplate consent form between a Data Owner (Consumer) and Enterprise.
See below the boiler plate template for Consent form :
I, [Data Owner's Full Name], provide my consent to [Enterprise's Full Name] ("Enterprise") for the collection, processing, and sharing of my personal data. I acknowledge and agree to the following:
Purpose: The Enterprise will collect and use my data for the following purpose(s): [Specify the purpose(s) in simple terms].
Data Categories: The types of personal data that may be collected include [Specify the categories of data in everyday language, e.g., name, email address, contact details, Data Attribute Categories..].
Consent Duration: This consent is valid for [Time Duration], and I reserve the right to withdraw my consent at any time.
Ownership: I understand that I retain ownership of my personal data, and the Enterprise will use it solely for the specified purpose(s).
Data Sharing: My personal data may be shared with trusted third parties, as required, but always in compliance with applicable data protection laws and regulations. Additionally, it is mandatory that I am notified every time my data is shared with any third party.
Security Measures: The Enterprise will implement appropriate security measures to safeguard my personal data from unauthorized access, alteration, disclosure, or destruction.
Data Subject Rights: I have the right to access, rectify, erase, restrict, and port my personal data. I can exercise these rights by contacting the Enterprise as provided below.
Complaints and Grievances: If I have any concerns or complaints regarding the processing of my personal data, I can reach out to the designated grievance officer at the following contact details: [Provide contact details of the grievance officer or relevant department].
Data Retention: The Enterprise will retain my personal data only for the duration necessary to fulfill the specified purpose(s) or as required by law.
Jurisdiction and Applicable Laws: This consent form is governed by the laws of [xx jurisdiction], and any disputes arising from or related to this consent form shall be subject to the exclusive jurisdiction of the courts in [XX Country].
By signing below, I confirm that I have read and understood the terms of this consent form. I willingly provide my consent to the Enterprise for the processing of my personal data as outlined above.
